PHYSICALS
Physicals are required each year for all incoming 9th - 12th grade band students.
Hometown Medical Associates will be on-site during the summer giving $25 physicals. Date TBD

Here are a couple of clinics in town that are offering physicals @ $25 if you can't make it during our summer date.
Corsicana Specialty Care Clinic @ 220 East 3rd Ave.
Hometown Medical Associates - 106 S Beaton St, Corsicana, TX 75110
LBM Medical Clinic - 829 N Main Street,- Located inside the P&S Pharmacy, across from Brookshires Grocery.
Physicals prior to May 2025 will not be valid for the 2025-2026 School Year.
Please be sure you turn in your Physicals and Medical History to your coach, band director, or to one of the Athletic Trainers.

Parents can login to Rank One to complete all on-line student forms.
All Athletic Forms that are required will be submitted electronically with the exception of the UIL Pre-Participation Physical and Medical History.
The student's 6 digit ID number will be required to complete forms.
Click the link for a tutorial that will walk you through the process.
All online forms must be completed before any student athlete will be allowed to practice or tryout for any athletic team.
